Authorization And Consent Form 

This Release Form Authorizes And Grants Your Consent To Be Tested For Drugs and Alcohol Both Before You Are Hired And After You May Be Hired On A Random Basis Or For Reasonable Suspicion.

Further, it signifies your Consent to allow a testing laboratory or other facility to take a urine, hair or blood sample and submit the results for a pre-employment, post-employment, random or reasonable suspicion drug test and to provide the results to the prospective or current employer.

Indemnity Agreement And Agreement Not To Sue

In consideration of these testing services being conducted on my behalf, I understand the result of such drug tests will be released to the company requesting the test, and I AGREE AND CONSENT NOT TO FILE ANY LAWSUIT OR OTHER ACTION against:

Hold Harmless And Indemnity Agreement

In connection with the results of such screening or testing being made available, I hereby agree to HOLD HARMLESS AND INDEMNIFY (Employer Company Name)  as well as  the laboratory or other drug testing service, the agents, employees or officers of the laboratory, medical facility or other testing service from all, reasonable attorney’s fees, damages, expenses and court costs which they or any of them may incur, jointly or separately.
