Please fill out and sign the application forms completely.
In order to insure to insure the protection of an individual’s personal credit information and history, Strict Federal requirements have been imposed on all three Credit Bureaus.
In that connection, they are required to adhere to strict verification rules as outlined in the application before individual consumer credit information can be accessed by employers.
In addition to the application forms, Trans Union will also require a copy of your company’s business license or the equivalent along with a letter of intent written on company letter head stating what your organization intends to use the credit reports for (Federal law only allows an employment credit report to be used for an employment related decision) Any clients who have had a site inspection for credit reports within the last 24 months will not be required to have a new one unless advised otherwise.
In order to insure your access to credit reports, Trans Union will require these documents to be submitted to them no later than __________
